Less than for most peptides, for a structural reason: the lactam bridge that makes this molecule superpotent also makes it chemically quiet, so the question is what the ring does not protect.

The 23-membered side-chain lactam joins the beta-carboxyl of Asp5 to the epsilon-amine of Lys10, so the aspartate side chain cannot fold back onto the backbone and the succinimide and isoAsp chemistry that dominates linear peptide degradation is suppressed. The N-terminus is acetylated, so there is no free amine and therefore no pyroglutamate route and no diketopiperazine cleavage. Add the absence of cysteine, methionine, asparagine, glutamine, tyrosine and serine, and disulfide scrambling, thioether oxidation and deamidation go too.

Three routes remain. Tryptophan is the first. It oxidises to hydroxytryptophan at plus 16 daltons, to N-formylkynurenine at plus 32 and to kynurenine at about plus 4, and photo-oxidises under near-ultraviolet light. The kynurenine-type products absorb in the near-visible, so yellowing is a visible indicator of a chemical event and the only degradation signal here that needs no instrument.

Histidine is the second. Metal-catalysed oxidation converts it to 2-oxo-histidine, a route needing only trace copper or iron plus dissolved oxygen. Lan and colleagues measured its pKa at 6.54 in 1994, so the imidazole changes protonation state across the ordinary formulation window of pH 5 to 8, and with it the peptide’s net charge and adsorption behaviour. Arginine at pKa 11.72 stays cationic throughout.

The third route is the interesting one. The C-terminal lysinamide slowly hydrolyses to the free acid, and that free acid is bremelanotide: C50H68N14O10 at 1025.2, one dalton heavier, identified as a metabolite in the original Arizona trials and approved by FDA in 2019 as Vyleesi. A solution left long enough at the wrong pH does not simply lose potency. It converts, slowly, into a different melanocortin agonist with a different receptor preference, and the 1 dalton difference is one no routine assay of a research preparation resolves.

Physical behaviour is less settled. The molecule is amphipathic, with a log P of 2.82 at pH 7.35 and a net charge of roughly plus 1 to plus 2 across pH 4 to 6, the classic profile for self-association and for adsorption to glass and polyethylene. Nobody has measured self-association at the 3.33 mg/ml of a melanotan 2 pen. The nearest evidence is that Vyleesi is formulated at 5.83 mg/ml in 2.5 percent glycerin with no surfactant.

Melanotan 2 Solution Stability: What Has Been Studied

No forced-degradation study and no long-term aqueous stability study of this compound has ever been published, which is a striking gap for a peptide first described in the late 1980s.

What exists is preformulation work: Lan and colleagues reported pKa values, partition coefficients and 4.6 percent oral bioavailability in rats in 1994, and Kilpelainen and colleagues loaded the peptide into porous silicon microparticles at 15 percent by weight in 2011. Neither addressed melanotan 2 solution stability over months in a sealed container, which is what a preloaded device raises.

The most useful indirect evidence comes from the approved relative. Vyleesi holds 1.75 mg in 0.3 ml, a ready-to-use solution formulated with 2.5 percent glycerin and water with hydrochloric acid or sodium hydroxide for pH adjustment, stored at or below 25°C, not frozen, protected from light. Wessells and colleagues followed with a double-blind placebo-controlled crossover study in men with psychogenic erectile dysfunction in 1998, and a further crossover study in 20 men reported in 2000: erection in 17 of 20 without sexual stimulation, a mean 41 minutes above 80 percent rigidity, increased desire after 68 percent of active administrations against 19 percent of placebo, and severe nausea after 12.9 percent of administrations at 0.025 mg per kilogram.

There is no phase II or phase III programme, no registered trial and no NCT number. Rodent work is far more extensive, covering feeding, thermogenesis, insulin sensitivity and erectile function, with PubMed listing 55 titles containing the compound name. Controlled pigmentation efficacy, long-term safety, prospective melanoma risk and the stability of a multi-dose device have none of them been studied.

The case literature is where the harm signals sit, and it reads as data rather than as warning labels. Schulze and colleagues reported eruptive naevi and darkening of existing naevi within 24 hours of a single injection in 2014, Hjuler and Lorentzen melanoma associated with use in the same year, Nelson and colleagues rhabdomyolysis and acute kidney injury after a 6 mg injection in 2012, Devlin and colleagues priapism after overdose in 2013, and Peters and colleagues renal infarction in 2020. Case reports of oral mucosal changes with intranasal products appeared in 2025 and 2026.

Safety and Regulatory Position

This compound has never been approved by FDA, EMA, PMDA, TGA or Health Canada, and FDA has stated its position on the record in an enforcement document rather than only in guidance.

Federal Register document 2016-27244, published 14 November 2016, sets out a debarment order against Edward Manookian, owner of Melanocorp Inc. of Tennessee, convicted on 28 August 2015 in the Middle District of Tennessee on two counts of conspiracy under 18 USC 371 for shipping this peptide sold online as an injectable tanning product after assuring FDA he would stop. The order records FDA’s position that the substance constituted a new drug under the Federal Food, Drug, and Cosmetic Act that could not enter interstate commerce without an approved application. Multiple national agencies have issued warnings about unregulated melanocortin products, reviewed by Habbema and colleagues in 2017 and by Adler and colleagues in Australia the same year.

It is not a component of an approved drug, has no USP monograph and is not on the 503A bulks list, so it is ineligible for compounding in the United States, and it was absent from the agenda of FDA’s Pharmacy Compounding Advisory Committee meeting of 23 and 24 July 2026. What Is the Mechanism of Action at the Melanocortin Receptors?

There are five: MC1R on melanocytes, MC2R for ACTH in the adrenal cortex, MC3R and MC4R in the central nervous system, and MC5R in exocrine glands. Skin pigmentation comes from MC1R. Agonism there switches the pigment cells from producing red-yellow pheomelanin to producing brown-black eumelanin, which is the whole of melanogenesis in this context, and increased melanin production is what produces the tanning effect the compound is sold for. Melanin production of that kind is a change in pigment type as much as in pigment quantity. What Do the Regulators and the Case Reports Say?

They have been saying the same thing for over fifteen years. The MHRA in the United Kingdom has repeatedly warned against unlicensed tan jabs, and tan jabs sold as injections or as nasal sprays have been the subject of seizure actions and public warnings there and in Ireland, Norway, Sweden, Denmark and Australia. Tan jabs bought online carry no assurance of identity, purity or sterility, and the tabloid nickname barbie drug attached to this compound has done nothing to slow that market.

The medical literature is a series of case reports rather than a trial programme, and its content is consistent. Reported side effects include nausea and vomiting after injection, facial flushing, headache, spontaneous penile erection in men, and darkening of existing moles. Those side effects come from individual case reports rather than from a trial, so their frequency is unknown. That last one is the finding that matters most. Multiple published reports describe rapid change in melanocytic naevi in users, and several describe the disease being diagnosed after use. Causation has not been established in a controlled study and cannot be from case reports alone, but the pharmacology gives the concern a mechanism: a compound that drives melanocyte activity is being used by people who also seek UV exposure, and UV exposure is the established melanoma risk factor.

Other reported harms include rhabdomyolysis, renal infarction and impairment, and posterior reversible encephalopathy syndrome.
